Preferred Name Added to Forms
There has been a change to the Appointment Form (PER-38) and Change or Separation Form (PER-39) for classified and unclassified employees. The Division of Human Resources has added the field “Preferred Name” to these two forms. This field gives the employee the opportunity of using a different first name for the Campus Phone Book and on-line White Pages.

When processing transactions, the name that is entered into HRIS must match the name that is on the social security card for paycheck and W-2 purposes. The “Preferred Name” gives the employee the option of adding a common name other than the name on the social security card. An example would be if an employee would prefer to use the name Bob instead of Robert or Chris instead of Christina. Another example would be an employee who goes by his or her middle name instead of first name; such as if an employee’s given name was Donald James, but the person commonly is referred to as James or Jim.

The “Preferred Name” provides easier identification for individuals looking in the Campus Phone Book or on-line White Pages. The name on the paycheck and W-2 will remain the same as the name that is entered from the social security card.
