Updated Benefits Forms
The following forms have been updated to provide the
employee an option of using either their Social Security Number or Employee
Identification Number when submitting these documents:
FMLA Employee Request Form
FMLA Employer Checklist
FMLA Designation of FMLA Leave
Application for Tuition Assistance (PER-32)
Application for President’s Scholarship (PER-32A)
US Savings Bond Payroll Savings Authorization (PER-14)
Additionally, the Accident/Injury Report (PER-17) has been changed to report the Employee Identification Number (e.g. W0000000000) instead of the Social Security Number.
The Division of Human Resources forms page has been updated to reflect these changes. Questions should be addressed to Benefits Administration in the Division of Human Resources at (785) 532-6277 or benadmin@ksu.edu.
