Leave Record forms are available for those full-time employees who want to maintain their own leave accruals, usage and balances.

Revised PPM Chapter 4720, Hourly Student Employees
The KSU Policy and Procedures Manual Chapter 4720, Hourly Student Employees has been revised effective January 29, 2003 regarding the addition of the Intellectual Property Agreement form required for submission when hiring hourly student employees. This also reflects the change of PER form numbers and names.

Revised PPM Chapter 3035, Inclement Weather General Policy and Procedure
The KSU Policy and Procedures Manual Chapter 3035, Inclement Weather General Policy and Procedure has been revised effective January 31, 2003 to more clearly state policy regarding the Declaration of Inclement Weather as it pertains to KSU employees and to clarify policy in the event that a declaration has not been issued.

Group Health Insurance Enrollments and Changes
A friendly reminder about some administrative procedures for processing Group Health Insurance Enrollment and Change Forms:

1. employee's desiring to enroll a family member must provide supporting documents establishing the relationship, e.g. marriage certificate, birth certificate.
2. employees desiring to add or delete a family member, other than during the annual open enrollment period, must have a qualifying mid-year event in order to do so. A list of these events may be found in the State of Kansas Active State Employee Benefits Guide at http://da.state.ks.us/ps/documents/benguide.pdf. The employee has 31 days from the date of the event to submit a Group Health Insurance Change Form for this addition or deletion along with appropriate supporting documents.
3. any family member over 60 days of age is required to have a Social Security Number (SSN). Employees have up to 60 days from the date of birth to provide the newborn's SSN.
4. the 31-day time limit for selecting or declining enrollment in the State of Kansas health insurance program begins from the effective date of employment indicated on the Appointment Form (PER-38) regardless of when the employee actually begins work or when a contract is signed.
5. submit all copies of the Group Health Insurance Enrollment Form and Group Health Insurance Change Form to the Division of Human Resources.
