Home Address
It is important that the Division of Human Resources (HR) has the correct home address and phone number on file for both current employees and those who have resigned. Paycheck advices, benefits information and employee W-2's are all sent to the employee's home address.
An individual who changes his/her address while employed with Kansas State University should contact the personnel specialist in his/her department with the new information. This also applies to those terminating employment with the university, since W-2's are sent to the most recent address on record with HR.
For Classified Staff and Unclassified Staff/Faculty, the department
personnel specialist will either change the address
on-line in the Human Resource Information System (HRIS), or forward an
Change or Separation Form (PER-39)
to HR. For hourly students and graduate
students, the department personnel specialist will either change the
address on-line in HRIS or forward an Hourly Student Data
Sheet (PER-40)
or a Change or Separation
Form (PER-39)

Work Address
Work address and phone number changes should be submitted to HR by
department personnel specialist. A Position Data Sheet
(PER-36)
should be completed with the appropriate
changes.
Each year, HR distributes Position Update Sheets to departments. Position changes can also be made by updating these sheets and submitting them to HR.
