KPERS Designation of Beneficiary (KPERS-7/99)

Upon the death of a benefits eligible university support staff or unclassified employee (except those participating in the Kansas Police and Firefighter's (KP&F) plan), regardless of cause, the insured death benefit provides the employee's named beneficiary 150 percent of the employee's annual rate of compensation.  The KPERS Designation of Beneficiary form may be used to name a primary and/or contingent beneficiary.
