| Workers’ Compensation Program | Initial notification (by telephone or e-mail) of injury or accident made to the Division of Human Resources as soon as possible. Submit Accident/Injury Report (PER-17) within 3 business days of injury or accident. Medical updates submitted to the Division of Human Resources as soon as possible. Advise (by telephone or e-mail) the Division of Human Resources immediately when employee returns to work if absent one full shift or more. |
